The Patient Implant Information Library is designed to help patients find additional information from their patient implant card. This may include warnings and precautions, information about the expected lifetime of the device, any necessary follow-up, and other information to ensure safe use of the device by the patient.

Not all products have information available on this website. Contact your healthcare provider to receive information about your implanted device.

How do I know which locale to select?

Searching for Documents

How do I search for my device information on this website?

First select your language. Then select the locale of the healthcare center where the procedure for your device was performed. The search field is shown at the top of the page after selecting your locale. Refer to your implant card to enter your device information.

Contact your healthcare provider if you are uncertain about what information you should insert into the search field or which document to select after searching. Incorrect selections will provide the wrong information about your product.

Where can I find the model number or product name of my device?

The device information is located on the patient implant card given to you by your healthcare provider.

If you do not have your implant card, call the healthcare provider where your procedure was performed for assistance.

What if I cannot locate the device information on the website?

First, verify that you have selected the locale of the surgical procedure. Next, double-check the model number or product name that you used for your search. If you still cannot find what you are looking for, please note that not all products and locales have information on the patient implant information library website. Contact your healthcare provider if you are unable to locate the device information you are looking for.

How do I view a document?

Once you see the search results for the product, click on the "View/Download" link near the document title to view it via your web browser. For best results, use Adobe® Reader® to view these documents. You can download the latest version of Adobe® Reader® for free.

How do I download a document?

Once you have viewed the document, you may download and save it by using the download function of the PDF viewer. Click the download or save icon of the PDF viewer. Follow the prompts to save the document to your computer.

If it is what you want to print, click the printer icon of the PDF viewer. Follow the prompts to print selected pages or the entire document.

Ordering Documents

How do I order a printed document?

Please note that not all documents are available for print ordering.

If ordering is available, first select your language. Then select the locale of the healthcare center where the procedure for your device was performed. The search field is shown at the top of the page after selecting your locale. Refer to your implant card to enter your device information.

Once you see the search results for the product, click on the "View/Download" link near the title of the document you want to order. View and verify it is the document you want to order. Navigate back to the search results by clicking on the previous browser tab. Next, click the "Order a Printed Copy" link near the title of the document. An order summary page will appear, which includes all documents in your current order. Once you select "CONTINUE", you will be prompted to complete an order form. Complete all required fields (marked with *). Click "CONTINUE" to confirm your information. Click "SUBMIT ORDER" to place your order.

How quickly will I receive my order?

You will receive orders for printed documents within seven business days. Note that the shipping schedule may be impacted by holidays.

What if there is an issue with my order or if I am having trouble ordering documents?

If you are not able to order a printed document, contact your healthcare provider. Also, remember that documents are available 24 hours a day, 7 days a week for electronic viewing, downloading, or printing on this website.
